If you see a charge on your credit card statement which you have not authorized or cannot identify, report it immediately by phone, using the number on the back of your card.
Confirm the report with a letter labeled “notice of billing error,” containing your name, address and at least last four digits of your card number, and addressed to the mail or email address provided for that purpose on your monthly statement. Send such a letter in addition to anything you were told to do over the telephone. Keep a copy.
The law expects you to review your monthly statements on a monthly basis and to notify the card issuer if you do not get one. You have not more than two billing cycles (generally 60 days) to provide notice of a billing error, and should do so as soon as possible.
